Loading dose: 5 mg/kg (150 mg – 300 mg) in 100 ml diluent [1,2].
Maintenance dose: 10 – 20 mg/kg/day (max: 1.2 g) in 250 – 500 [3,4] ml diluent
Ventricular fibrillation or pulseless ventricular tachycardia
Loading dose: 5 mg/kg (300 mg) diluted in 20 ml diluent. Add 2.5 mg/kg (150 mg) if necessary,
Followed by 900 mg in 500 ml over 24 hr

Remarks
Drug levels above 3 mg/ml in D5 has been linked to high incidence of peripheral vein phlebitis; levels of
2.5 mg/ml or lower are less irritating. Therefore, for infusions longer than 1 hour, do not exceed
concentrations of more than 2 mg/ml, unless using central venous catheter.
