PURPOSE

  1. To disconnect the patient from the machine on completion of dialysis.
  • To investigate malfunction in the system

POLICY

  1. All nursing staff working in the renal unit should be competent in disconnecting patient from (PD) peritoneal
  2. An experienced nurse should carry out this procedure.
  3. A strict aseptic technique should be maintained throughout the procedures in accordance with the policy and procedure of aseptic technique.
  4. Hand washing should be performed according to the policy and procedure on hand washing.

 

EQUIPMENT

  1. Trolley washed with soap and water and wiped with 70% spirit.
  2. On & Off pack
  3. Betadine solution (Antiseptic)
  4. Mepore dressing 9x15cms(2)
  5. OP site dressing 10x2cms (2)
  6. Sterile gloves
  7. Sterile clamp
  8. Andy Clamp
  9. Frekaderm hand rub
  10. Freksept spray
  11. Surgical Mask
  12. Disposable apron

PROCEDURE

01Ensure that the prescribed number of liters is completed.  Shows-that treatment completed.  
02Ensure that all fluid has been drained from the patients.  To prevent fluid overload.

03When the machine shows that P.D. Plus cycle Completed Disconnect Patient press.   
04Second Nurse washes and wipes hands and apply Frekaderm hands rub thoroughly.   
05Close the clamps on the patient line and the catheter extension and spray Frekasept over it (breaking point).   To prevent infection.
06Place the ANDY. Clamp between the patient line and the predetermined breaking point of the tubing set at the predetermined breaking point.  To prevent air entering to peritoneum.  
07In the machine press the key. Remove the tubing set from the upper clamps. Press the key remove the tubing set from the lower clamps. Press the key and dispose the tubing and put off the machine.   
08First Nurse Wears Disposable apron and surgical mask.   
09Clean the trolley with 70% spirit.To prevent infection.  
10Explain the procedure to the patient.   
11Place the patient in a comfortable position.   
12Hand washing should be done as per hand washing policy and procedure and wears sterile gloves.   
13Open the ON& OFF pack. Second Nurse pours beta dine solution into the galipot   
14First Nurse does the dressing according to the policy and procedure.   
15Secure the catheter with dressing.   
16Wash and dry hands.  To prevent infection.
17Check vital signs.To assess patient’s condition.  
18Document the procedure in the Nursing Notes. Administer prescribed medication.
