Patient Health Questionnaire-9

PHQ-9 Depression Self-Test: Overview and Interpretation

The Patient Health Questionnaire-9 (PHQ-9) is a widely used 9-item self-assessment tool for screening and measuring the severity of depression. It is based on the DSM-5 criteria for major depressive disorder (MDD) and is commonly used in clinical and research settings.

Key Features of the PHQ-9

  • 9 questions rated on a 4-point scale (0–3):
    • 0 = Not at all
    • 1 = Several days
    • 2 = More than half the days
    • 3 = Nearly every day
  • Assesses symptoms over the past 2 weeks.
  • Quick & reliable: Takes <5 minutes, validated in multiple studies.

Scoring & Interpretation

  • Total Score Range: 0–27
    • 0–4 = Minimal or no depression
    • 5–9 = Mild depression
    • 10–14 = Moderate depression
    • 15–19 = Moderately severe depression
    • 20–27 = Severe depression

A score ≥10 suggests clinical depression and warrants professional evaluation.

Why Use the PHQ-9?

  • Clinically validated: Used by doctors and therapists worldwide.
  • Tracks depression severity: Helps monitor treatment progress.
  • Identifies suicide risk: Question 9 flags urgent concerns.

Limitations

  • Self-report bias: Depends on honest responses.
  • Not a diagnosis: Only a screening tool—requires clinical follow-up.
  • Doesn’t assess causes: Doesn’t differentiate between biological, situational, or trauma-related depression.
