{"id":586,"date":"2022-04-08T18:58:26","date_gmt":"2022-04-08T18:58:26","guid":{"rendered":"https:\/\/kidneydiseaseclinic.net\/blog\/?p=586"},"modified":"2022-04-08T18:58:27","modified_gmt":"2022-04-08T18:58:27","slug":"haemodialysis-hazards-breakdown-during-haemodialysis","status":"publish","type":"post","link":"https:\/\/kidneydiseaseclinic.net\/blog\/haemodialysis-hazards-breakdown-during-haemodialysis\/","title":{"rendered":"HAEMODIALYSIS HAZARDS &#8211; BREAKDOWN DURING HAEMODIALYSIS"},"content":{"rendered":"\n<h1 class=\"wp-block-heading\">PURPOSE<\/h1>\n\n\n\n<p>1.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; To provide guidelines regarding exchanging machine during dialysis.<\/p>\n\n\n\n<p>2.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; To complete dialysis session without any complication.<\/p>\n\n\n\n<p>3.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; To demonstrate safe and skilled technique of handling dialysis.<\/p>\n\n\n\n<p><strong>CONDITIONS<\/strong><\/p>\n\n\n\n<p>1.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Two (2) nurses, of whom one should be an experienced nurse should carry out this procedure.<\/p>\n\n\n\n<p>2.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; A spare machine should be kept ready at all time for use in case of emergency, if available.<\/p>\n\n\n\n<p>3.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; All Registered Nurses working in renal unit must be aware of nursing responsibility in case of machine breakdown during dialysis.<\/p>\n\n\n\n<p>4.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; The functional test in the machine must be completed prior to start each dialysis session.<\/p>\n\n\n\n<p>5.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; This procedure should be carried out within 15-20 minutes to prevent complications.<\/p>\n\n\n\n<p><strong>PROCEDURE<\/strong><\/p>\n\n\n\n<figure class=\"wp-block-table\"><table><tbody><tr><td>SN<\/td><td>Nursing Action<\/td><td><strong>Rationale<\/strong><\/td><\/tr><tr><td>01<\/td><td>As soon as the machine breakdown, the nurse should call for help from another nurse\/ technician. &nbsp;<\/td><td>&nbsp;<\/td><\/tr><tr><td>02<\/td><td>Switch OFF the machine after writing down the parameters on the machine including time. &nbsp;<\/td><td>&nbsp;<\/td><\/tr><tr><td>03<\/td><td>Release \u201cV\u201d air detecor clamp. &nbsp;<\/td><td>&nbsp;<\/td><\/tr><tr><td>04<\/td><td>Rotate the blood pump manually. &nbsp; <strong>Alert<\/strong> <strong>All nurses need to be very vigilant because there are no machine alarms.<\/strong> &nbsp;<\/td><td>&nbsp;<\/td><\/tr><tr><td>05<\/td><td>Explain the procedure to the patient and re-assure him\/her. &nbsp;<\/td><td>To allay fears and gain patient\u2019s confidence and cooperation.<\/td><\/tr><tr><td>SN<\/td><td>Nursing Action<\/td><td><strong>Rationale<\/strong><\/td><\/tr><tr><td>06<\/td><td>If no spare machine return the blood manually . &nbsp;<\/td><td>&nbsp;<\/td><\/tr><tr><td>07<\/td><td>Bring the spare machine to the bedside and switch on (same company). &nbsp;<\/td><td>&nbsp;<\/td><\/tr><tr><td>08<\/td><td>Disconnect water inflow and drain connection from the other machine and connect to spare machine. Put concentrate stick in the dialysate fluid and start functional test. &nbsp;<\/td><td>&nbsp;<\/td><\/tr><tr><td>09<\/td><td>Once the spare machine is ready, clamp the arterial and venous pressure monitors, patient lines, and heparin line. &nbsp;<\/td><td>&nbsp;<\/td><\/tr><tr><td>10<\/td><td>Disconnect dialysate lines from the dialyzer and replace them in lock. &nbsp;<\/td><td>&nbsp;<\/td><\/tr><tr><td>11<\/td><td>With the assistance of the other nurse, remove gently the dialyser and tubing from the faulty machine and connect to the spare machine. &nbsp; <strong>Alert<\/strong> <strong><em>Ensure that the catheter \/ fistula needles are in sites and the tubing are not pulled while transferring.<\/em><\/strong> &nbsp;<\/td><td>To recommence safe dialysis.<\/td><\/tr><tr><td>12<\/td><td>Attach dialysate lines to the dialyser. &nbsp;<\/td><td>&nbsp;<\/td><\/tr><tr><td>13<\/td><td>Fix arterial and venous lines and release all clamps. &nbsp;<\/td><td>&nbsp;<\/td><\/tr><tr><td>14<\/td><td>Reset all alarms. &nbsp; <strong>Alert<\/strong> <strong><em>Always check the system for air \/ blood clot.<\/em><\/strong> &nbsp;<\/td><td>&nbsp;<\/td><\/tr><tr><td>15<\/td><td>Check all dialysis parameters. &nbsp;<\/td><td>&nbsp;<\/td><\/tr><tr><td>16<\/td><td>Start haemodialysis and adjust the remaining dialysis time. &nbsp;<\/td><td>To complete the required hours of dialysis.<\/td><\/tr><tr><td>17<\/td><td>Take and record vital sings &nbsp;<\/td><td>To monitor the condition of the patient. &nbsp;<\/td><\/tr><\/tbody><\/table><\/figure>\n\n\n\n<figure class=\"wp-block-table\"><table><tbody><tr><td>SN<\/td><td>Nursing Action<\/td><td><strong>Rationale<\/strong><\/td><\/tr><tr><td>18<\/td><td>Document the incident including the time, when the machine breakdown, time taken when transferring, time restarted, and presence \/ absence of blood clot in the system. &nbsp;<\/td><td>&nbsp;<\/td><\/tr><\/tbody><\/table><\/figure>\n","protected":false},"excerpt":{"rendered":"<p>PURPOSE 1.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; To provide guidelines regarding exchanging machine during dialysis. 2.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; To complete dialysis session without any complication. 3.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; To demonstrate safe and skilled technique of handling dialysis.
